- <p>Background: Unhealthy alcohol use may lead to arrhythmia and cardiomyopathy, but its impact on sudden death is not well understood.</p> <p>Objective: To investigate the association of unhealthy alcohol use with sudden death.</p> <p>Methods: We conducted a case-control study in Wake County, a large (~1 million inhabitants), diverse county in North Carolina. We screened and adjudicated victims of sudden, unexpected, out-of-hospital deaths in adults aged 18–64 years reported by emergency medical services between 2013 and 2015. We randomly selected sex- and age-matched control patients from a university health system from the same county and time period. Characteristics of sudden death victims and controls were ascertained via standardized chart reviews. Unhealthy alcohol use was identified via chart review and was defined as any evidence of excessive alcohol use, such as it being stated in the social history or medical history, alcohol abuse being listed as a possible contributor to death, or alcohol-related diagnoses. We used logistic regression to estimate odds ratios (ORs) for the association of unhealthy alcohol use and sudden death, adjusting for age, sex, race, and other psychiatric diagnoses, including depression, anxiety, schizophrenia, bipolar disorder, and substance use disorders other than tobacco and alcohol. We also calculated the E-value to estimate the impact of any unmeasured confounders.</p> <p>Results: We identified 399 sudden death victims, of whom 374 (94%) had alcohol use data available. Among these 374 included victims, 256 (68%) were male, and 239 (62%) were White, with a median age at death of 55 years (IQR 48, 60). The demographic characteristics of the 1114 matched controls were similar to those of sudden death victims. Unhealthy alcohol use was present in 115 (31%) sudden death victims and 27 (2%) controls. In analyses adjusted for demographics only, unhealthy alcohol use was associated with a higher incidence of sudden death, with an OR of 17.5 (95% CI 11.4, 27.8). When further adjusted for other psychiatric diagnoses, the OR was 11.2 (95% CI 7.1, 18.0). The calculated E-value was 21.8, meaning an unmeasured confounder would need to be associated with both unhealthy alcohol use and sudden death by 21.8-fold to explain away the observed OR.</p> <p>Conclusions: Unhealthy alcohol use was strongly associated with higher sudden death risk in working-age adults. Our calculated E-value indicates it is unlikely that any unmeasured confounders alone would account for the observed association. Our findings suggest that interventions to reduce unhealthy alcohol use may be an effective strategy to prevent sudden death in working-age adults.</p>

- This study aims to evaluate the quantitative impact of FDG-PET on detecting extra-pelvic metastases in women with cervical cancer, using population-based data, following its approval by US Medicare/Medicaid in 2005 for staging patients without detectable extra-pelvic metastases on CT or MRI scans. Surveillance, Epidemiology, and End Results (SEER) data from eight US registries from 1975 to 2021 were analyzed. The proportions of synchronous metastases “Synchronous/(Synchronous + Metachronous)” were compared between the pre-PET era (1995–1999 and 2000–2004) and the PET era (2010–2014). The estimated fraction of extra-pelvic metastases at initial diagnosis was 35% in the pre-PET era (2000–2004) compared to 46% in the PET era (2010–2014) (absolute difference 11%; p < 0.001). The proportion of undetected metastases decreased from 65% to 54%, indicating that PET identified up to 17% (i.e., [65 − 54]/65) of previously undetectable subclinical metastases. Likewise, the comparison of data from 1995–1999 vs. 2010–2014 showed 29% vs. 46% (absolute difference 17%; p <0.001). The proportion of undetected metastases decreased from 71% to 54%, suggesting that PET identified up to 24% (i.e., [71 − 54]/71) of the previously undetectable subclinical extra-pelvic metastases. In conclusion, up to ≈17–24% of subclinical extra-pelvic metastases, previously undetectable at initial diagnosis, appear to be identified through PET, findings that are consistent with and externally validate our prior population-based observations in non-small cell lung cancer.

- Intraperitoneal (I.P.) delivery of cell-based therapeutics represents a promising strategy for treating regional peritoneal diseases; however, rapid cellular clearance severely limits therapeutic durability. A critical unmet need is the development of implantable biomaterial platforms that can both mechanically integrate within the dynamic I.P. cavity and sustain viable cell persistence in vivo. Here, we establish a Continuous Liquid Interface Production (CLIP)-based 3D bioprinting strategy to engineer transplantable, cell-laden hydrogel scaffolds optimized for I.P. implantation. Through systematic bioresin design, we identify a GelMA-PEGDA formulation that achieves a balance between high-resolution printability, tissue-matched mechanical characteristics (Young’s modulus 10–15 kPa), and controlled biodegradation (~75% mass loss over 14 days). The resulting constructs support sustained cell viability and proliferation for over 30 days in vitro. Importantly, in an animal study conducted in 6–8 weeks of female nude mice, in vivo I.P. implantation demonstrates a ~10-fold extension in cellular persistence compared to direct cell injection, prolonging the time to 50% signal decay from ~3 days to ~30 days, with detectable cell retention approaching two months in select animals. The platform further accommodates multiple clinically relevant cell types, including human mesenchymal stem cells and neural stem cells, highlighting its translational versatility. Collectively, this work defines key material and architectural parameters required for I.P. implantable cell therapeutics and establishes CLIP-based bioprinting as a scalable strategy for regional delivery of living therapeutics.

- Background: Assessing cortical and muscle activity simultaneously during task performance may inform motor function post-stroke. This study evaluated brain–muscle functional connectivity (corticomuscular coherence, CMC) in early stroke recovery. Methods: Individuals with stroke in an inpatient rehabilitation facility (IRF) completed motor assessments and simultaneous electroencephalography (EEG) and electromyography (EMG) recordings during a grip task at IRF admission and discharge. Beta (20–30 Hz) CMC was measured between EEG electrodes overlying the primary motor cortex (M1) and supplementary motor area (SMA) and EMG leads overlying the first dorsal interosseous (FDI). Neurotypical controls completed identical EEG/EMG recordings. Correlational analyses were performed to ascertain CMC and motor assessment associations. CMC differences by Group (Stroke vs. Controls), Time (Admission vs. Discharge), and Extremity (Affected/Dominant vs. Less Affected/Non-Dominant) were estimated using mixed-effects linear models. Results: Thirty individuals with stroke (14 females, mean age 67.0 ± 9.8 years, 10.4 ± 3.5 days post-stroke) and 17 controls (8 females, mean age 75.3 ± 13 years) participated. Individuals with stroke exhibited reduced beta CMC between SMA and affected FDI (F(1,36.1) = 5.73, p = 0.02, Cohen’s f = 0.40) compared to controls, with lower CMC involving the affected vs. less affected extremity (F(1,73.0) = 5.72, p = 0.01, Cohen’s f = 0.28). Greater beta SMA–FDI CMC at admission related to poorer motor recovery (ρ = −0.59, p = 0.01). Group and Extremity CMC differences were not observed over time, nor were there changes in affected extremity CMC from admission to discharge. Conclusions: Beta SMA–FDI CMC is a marker of neural injury, exhibiting extremity-specific differences early post-stroke. While beta SMA–FDI CMC correlated with motor recovery, the absence of change over time during hospitalization necessitates longitudinal assessments to clarify its trajectory alongside recovery.

- <p>Purpose: To develop a fully automated 2D nnU-Net pipeline for multi-class skeletal muscle segmentation (psoas, paraspinal, and abdominal wall) at the third lumbar (L3) vertebral level, and to quantitatively evaluate its diagnostic performance and reliability compared to manual segmentation.</p> <p>Materials and Methods: A 2D nnU-Net was trained on 164 axial L3 CT slices from the multi-institutional AMOS22 dataset, spanning diverse abdominal pathologies and multivendor imaging. To assess generalizability under severe anatomical distortion, independent external validation was performed in 50 consecutive patients with advanced liver disease from a single institution (January–December 2025; mean age, 63 ± 15 years; 32 women, 18 men), of whom 88% had moderate-to-severe ascites. Model stability was examined by comparing a five-fold ensemble with the best-performing single-fold model. Intra-observer reliability of the manual reference standard was evaluated in a random subset of 30 cases. Inter-observer agreement was additionally assessed using an independent second reader. Performance metrics included the Dice Similarity Coefficient (DSC), Pearson correlation coefficient (r), and Bland–Altman analysis for cross-sectional areas and mean attenuation. The inference workflow was deployed via a custom Streamlit-based graphical user interface (GUI).</p> <p>Results: In this anatomically complex external validation cohort, the 5-fold ensemble 2D nnU-Net achieved an overall mean DSC of 0.937 ± 0.043 (95% CI, 0.925–0.950), with 80% of cases achieving a mean DSC ≥ 0.90. While the mean DSC was statistically comparable to the best single-fold model (0.937, [95% CI, 0.921–0.952], p = 0.736), the ensemble strategy increased the minimum observed DSC (worst-case performance) from 0.720 to 0.822. Class-specific external validation performance for the 5-fold ensemble was highest for the paraspinal muscles (DSC: 0.960; 95% CI, 0.952–0.967), followed by the psoas muscles (DSC: 0.941; 95% CI, 0.927–0.956), and lowest for the anatomically complex abdominal wall muscles (DSC: 0.911; 95% CI, 0.893–0.929). Comparison between the ensemble model and manual segmentation yielded a Pearson correlation of r = 0.955 (p < 0.001) for total skeletal muscle area, with a mean bias of +7.17 cm2. Intra- and inter-observer agreements for the manual reference standard demonstrated correlation coefficients of r = 0.995 and 0.090 for total areas, respectively. The automated pipeline required 3–5 s per case for inference and quantitative reporting, compared to 3–5 min for manual segmentation.</p> <p>Conclusions: In patients with advanced liver disease and substantial anatomical distortion from ascites, an ensemble-based 2D nnU-Net provides high quantitative agreement with manual L3 skeletal muscle segmentation, while mitigating lower-bound (worst-case) errors relative to single-fold models. Integration with a dedicated GUI enables substantial time savings and supports scalable quantitative body composition measurement.</p>

- In Burke County, North Carolina, a Hepatitis A outbreak among unsheltered residents exposed gaps in access to clinic-based care and prompted early, ad hoc “backpack medicine” outreach efforts to deliver care directly in nontraditional settings. While this approach addressed immediate needs, it highlighted the inadequacy of isolated interventions, prompting local partners to pursue more structured, coordinated, and community-driven approaches to homeless health system design. This project report describes how Burke County Public Health, in partnership with the University of North Carolina at Chapel Hill, applied systems thinking, community coalition building, and human-centered design to transition from reactive outreach to a structured, sustainable mobile health delivery model for people experiencing homelessness. Guided by Community Coalition Action Theory (CCAT), partners used human-centered design methods to engage over 40 community stakeholders and 10 individuals with lived experience of homelessness or housing instability. Through empathy mapping, iterative prototyping, and thematic analysis, the team identified priority service gaps, defined operational requirements, and developed prototype service models, while building cross-agency readiness for implementation.

- This study reports, for the first time, changes in the microbiome community associated with anhydrobiosis in two tardigrade species of the genus Paramacrobiotus. To identify bacteria linked to the anhydrobiosis phenomenon and to track microbiome changes under anhydrobiotic stress, next-generation sequencing of bacterial 16S rRNA genes was conducted. Microbiome profiling was performed across various developmental and physiological stages of tardigrades, including: eggs; active adult specimens (both before and after 7, and 120 days of anhydrobiosis, referred to as short- and long-term anhydrobiosis, respectively); specimens in the desiccated tun stage; dead specimens following long-term anhydrobiosis (no dead specimens were observed after short-term anhydrobiosis); and the culture medium. It was shown that the microbiome community varied among stages, with high stage-specificity. Several bacterial genera were identified that may assist the host during anhydrobiosis, potentially through biofilm formation and by supporting stress-protective mechanisms such as heat shock protein expression and trehalose synthesis in eggs and tuns. These findings reveal that microbiota may contribute to anhydrobiotic survival in tardigrades, providing novel insights into host–microbe interactions under extreme environmental stress.

- <p>Anemia in aging is multifaceted and may be in part caused by increased inflammation and oxidative stress. Almonds contain nutrients that may improve inflammation or antioxidant activity, which can improve red blood cell (RBC) integrity and mitigate anemia. The aim of this study is to investigate the effects of almond consumption on hematologic markers in a mouse model of aging. Forty-eight 18-month-old male and female C57BL/6 mice were randomized to receive a control diet or an almond-supplemented diet (15% calories from almond meal) for 12 or 21 weeks (four groups, n = 12/group). Blood and tissues were collected after respective intervention periods for analyses of hematologic markers (complete blood count analysis) and antioxidant status (RBC alpha-tocopherol concentration and superoxide dismutase activity). Mice consuming the almond-supplemented diet had significantly lower red cell distribution width at 12 weeks (p = 0.05) and significantly higher RBC count at 21 weeks (p = 0.04). Regression analyses indicated that almond intake resulted in higher RBC count (β = 0.82, p = 0.026) and lower odds of anemia (OR = 0.20, p = 0.046) after adjusting for sex of mice and length of intervention. These findings suggest that almond intake in aging mice improved selected hematologic markers and reduced anemia risk.</p>

- <p>The quality of technology integration in K-8 science classrooms has significant implications for educational equity, particularly in minority–majority districts where teacher practice is among the strongest predictors of STEM persistence among underserved populations. This study examined the extent to which K-8 science teachers’ technology integration knowledge translated into transformative instructional practice within a large, minority–majority district in the U.S, using the frameworks of Information and communication technology (ICT)-Technological Pedagogical Content Knowledge (TPACK) and Passive, Interactive, Creative, Replacement, Amplification, Transformative (PICRAT) model. Technology integration planning knowledge was assessed using the ICT-TPACK instrument across elementary and middle school teachers. Instructional practice was rated using the PICRAT framework applied to teachers’ open-ended descriptions of their technology use. These responses also provided contextual illustration of quantitative patterns. Results indicate that while middle school teachers demonstrated significantly higher ICT-TPACK planning knowledge, this advantage primarily reinforced foundational science concepts through passive consumption rather than facilitating student agency. PICRAT analysis revealed that technology use across all grade levels was dominated by Replacement and Amplification practices, while creative and transformative uses remained nearly absent. These findings reveal a persistent knowing–doing gap in which planning knowledge did not translate into transformative enacted practice. Implications for equity-focused professional development and structural supports moving K-8 science teachers toward more transformative technology integration are discussed.</p>

- <p>Measles represents a critical public health challenge due to its high transmissibility and the resurgence of outbreaks of the disease, linked to gaps in vaccination coverage. In this study, we analyze the epidemiological dynamics of the outbreak that began in Mexico in 2025 and focus on Jalisco as the epicenter during the first quarter of 2026. A descriptive, retrospective, ecological time-series analysis was conducted using official epidemiological surveillance data, correlating incidence with immunization strategies. The results reveal that 6681 cases were recorded in Jalisco, with the highest prevalence among adults aged 20 to 49 years (41.96%), which highlights the accumulated gaps in immunity coverage. The public health response, comprising the administration of three million doses, focusing on zero-dose children and vaccination blockades, contributed to a notable decrease in infections by epidemiological week 17 of 2026. It was concluded that the outbreak was controlled through intensive and coordinated intervention. However, the proximity of the 2026 International Federation of Association Football (FIFA) World Cup requires vaccination coverage to be maintained at over 95%, in addition to vigilance, to mitigate the risk of viral reintroduction and to safeguard global health security.</p>
