- The objective of this study is to determine whether key hospital-level financial and market characteristics are associated with whether rural hospitals merge. Hospital merger status was derived from proprietary Irving Levin Associates data for 2005 through 2016 and hospital-level characteristics from HCRIS, CMS Impact File Hospital Inpatient Prospective Payment System, Hospital MSA file, AHRF, and U.S. Census data for 2004 through 2016. A discrete-time hazard analysis using generalized estimating equations was used to determine whether factors were associated with merging between 2005 and 2016. Factors included measures of profitability, operational efficiency, capital structure, utilization, and market competitiveness. Between 2005 and 2016, 11% (n = 326) of rural hospitals were involved in at least one merger. Rural hospital mergers have increased in recent years, with more than two-thirds (n = 261) occurring after 2011. The types of rural hospitals that merged during the sample period differed from nonmerged rural hospitals. Rural hospitals with higher odds of merging were less profitable, for-profit, larger, and were less likely to be able to cover current debt. Additional factors associated with higher odds of merging were reporting older plant age, not providing obstetrics, being closer to the nearest large hospital, and not being in the West region. By quantifying the hazard of characteristics associated with whether rural hospitals merged between 2005 and 2016, these findings suggest it is possible to determine leading indicators of rural mergers. This work may serve as a foundation for future research to determine the impact of mergers on rural hospitals.

- Objective: To investigate the influence of cochlear implant (CI) use on subjective benefits in quality of life in cases of asymmetric hearing loss (AHL). Study Design: Prospective clinical trial. Setting: Tertiary academic center. Subjects and Methods: Subjects included CI recipients with AHL (n = 20), defined as moderate-to-profound hearing loss in the affected ear and mild-to-moderate hearing loss in the contralateral ear. Quality of life was assessed with the Speech, Spatial, and Qualities of Hearing Scale (SSQ) pragmatic subscales, which assess binaural benefits. Subjective benefit on the pragmatic subscales was compared to word recognition in quiet and spatial hearing abilities (ie, masked sentence recognition and localization). Results: Subjects demonstrated an early, significant improvement (P <.01) in abilities with the CI as compared to preoperative abilities on the SSQ pragmatic subscales by the 1-month interval. Perceived abilities were either maintained or continued to improve over the study period. There were no significant correlations between results on the Speech in Quiet subscale and word recognition in quiet, the Speech in Speech Contexts subscale and masked sentence recognition, or the Localization subscale and sound field localization. Conclusions: CI recipients with AHL report a significant improvement in quality of life as measured by the SSQ pragmatic subscales over preoperative abilities. Reported improvements are observed as early as 1 month postactivation, which likely reflect the binaural benefits of listening with bimodal stimulation (CI and contralateral hearing aid). The SSQ pragmatic subscales may provide a more in-depth insight into CI recipient experience as compared to behavioral sound field measures alone.

- Satisfying intimate relationships are crucial to human health and well-being. Yet even the best relationships include good days and bad ones, and when people experience bad days in terms of relationship satisfaction, it tends to undermine personal well-being. What can reduce the extent to which bad relational days spill over into personal well-being? Based on the Buddhist concept of impermanence, as well as modern theory and research examining mindfulness, we argue trait mindfulness renders people more aware and accepting of all forms of change, including to changes in their relationships. As such, we hypothesized that people with greater trait mindfulness would be less likely to experience decrements in personal well-being on days in which they experienced dips in relationship satisfaction. In a daily study of 80 couples across 14 days (N = 1,798 observations), people experienced lower life satisfaction, greater negative emotions, and fewer positive emotions on days when they reported lower than their average relationship satisfaction, but this association was attenuated for people high in mindfulness. These results suggest trait mindfulness partially buffers the negative consequences of daily dips in relationship satisfaction.

- Cross-sectional and the empirical Bayes (EB) before–after are two of the most common methods for estimating crash modification factors (CMFs). The EB before–after method has now been accepted as one way of addressing the potential bias caused by the regression to the mean problem. However, sometimes before–after methods may not feasible because of the lack of data from before and after periods. In those cases, researchers rely on cross-sectional studies to develop CMFs. However, cross-sectional studies may provide biased CMFs through confounding. The propensity score (PS) matching method, along with cross-sectional regression models, is one of the methods that can be used to address confounding. Though PS methods are widely used in epidemiology and other studies, there are only a few studies that have used PS matching methods to estimate CMFs. The intent of this study is to evaluate and compare the performance of cross-sectional regression models using PS matching methods with the results from the EB and traditional cross-sectional methods. The comparisons were conducted using two carefully selected simulated datasets. The results indicate that optimal propensity score distance (PSD) matching with maximum variable ratio of 5 performed quite well compared with the EB before–after and the traditional cross-sectional methods.

- Gay, bisexual, and other men who have sex with men (MSM) are disproportionately affected by poor mental health compared to their heterosexual counterparts. One factor that may increase mental health problems among MSM is intimate partner violence (IPV) victimization. The objectives of this study are to (a) describe the prevalence of different forms of IPV victimization experienced by MSM living with HIV in Guatemala City and (b) examine the relationship between IPV victimization and mental health. We analyzed cross-sectional survey data from a cohort of MSM living with HIV in Guatemala City (n = 374) to describe the burden of IPV, including physical, sexual, and emotional IPV. We then examined relationships between lifetime IPV and each form of recent IPV (past 12 months) with self-reported anxiety and depression using multivariable logistic regression. Over a quarter (27.3%) of the participants screened positive for anxiety and nearly one fifth (17.9%) screened positive for depression. Over a quarter of the participants (28.6%) reported ever having experienced any IPV victimization and 8.8% reported having experienced any form of recent IPV. In multivariable analyses, participants who experienced any form of lifetime IPV had roughly twice the odds of experiencing anxiety (OR: 1.86; 95% CI = [1.03, 3.38]) and depression (OR: 2.02; 95% CI = [1.02, 3.99]) compared to those who had not. Participants who experienced recent emotional IPV had over seven times the odds of experiencing anxiety (OR: 7.23; 95% CI = [1.46, 38.85]) compared to those who had not. MSM living with HIV in Guatemala experience a high burden of anxiety, depression, and IPV victimization. Those participants who had experienced lifetime IPV and recent emotional IPV were significantly more likely to screen for anxiety and depression. To improve their mental health, HIV clinics and other health services should provide support for MSM who have experienced IPV victimization.

- In the present study we consider whether certain pre-service teachers (PSTs) particularly benefit from high-quality student teaching experiences. To conduct these analyses, we connect student teaching and K-12 workforce data for six educator preparation programs (EPPs) and assess whether placement school and cooperating teacher characteristics predict the effectiveness of early-career teachers. Results show that high-quality student teaching placements especially benefit PSTs with lower GPAs and narrow effectiveness gaps between teachers with lower versus higher GPAs. These findings call for closer partnerships between EPPs and school districts and suggest that EPPs may wish to prioritize placements for PSTs with lower GPAs.

- Philanthropic support of higher education is a growing area of interest among academic fundraisers and philanthropy scholars. The academic fields of science, technology, engineering, and math (STEM), in particular, are in need of a better understanding of their major donors. This article analyzes a unique database of announced gifts to higher education institutions from 1995 to 2017 to investigate relationships between major donors’ characteristics and the magnitude of their gifts to STEM and all other academic disciplines. Major donors to STEM are disproportionately entrepreneurs who, on average, give larger gifts to STEM than other major donors. Quantile regressions reveal a positive and statistically significant relationship between major donors’ entrepreneurial status and gift amounts at the 99th quantile (worth US$100 million or more). As major funding sources for academic STEM are increasingly threatened, these findings are pertinent to academic institutions seeking to leverage major donors as an alternative source of funding.

- Many workers have been drawn to the gig economy by the promise of flexible, autonomous work, but scholars have highlighted how independent working arrangements also come with the drawbacks of precarity. Digital platforms appear to provide an alternative to certain aspects of precarity by helping workers find work consistently and securely. However, these platforms also introduce their own demands and constraints. Drawing on 20 interviews with online freelancers, 19 interviews with corresponding clients and a first-hand walkthrough of the Upwork platform, we identify critical literacies (what we call gig literacies), which are emerging around online freelancing. We find that gig workers must adapt their skills and work strategies in order to leverage platforms creatively and productively, and as a component of their ‘personal holding environment’. This involves not only using the resources provided by the platform effectively, but also negotiating or working around its imposed structures and control mechanisms.

- This article explores the relationship between personal sacrifice and identity work within conditions of profound structural insecurity. We develop the concept of sacrificial labour to describe how individual self-sacrifice aligns workers’ identities to the needs of organizations while gradually foreclosing the actualization of individuals’ desired future selves. Drawing upon qualitative data from a longitudinal study of healthy individuals who enrol in paid clinical trials for the pharmaceutical industry, we make two contributions to the identity-work literature. First, we argue that the ongoing project of building stable and secure identities may become damaging when structural and cultural conditions defy even provisional, fragile attainment of this goal. Second, we reflect on how racialization and social marginalization erode identities and constrain possibilities for identity recuperation. Whereas the identity-work literature often focuses on the agential accomplishments of individuals, we provide a troubling account of how persistent social and economic inequalities confound identity realization efforts.

- Female sex workers (FSWs) have a notably high risk of acquiring human papillomavirus (HPV) infections. Relatively few studies address the type-specific prevalence and incidence of HPV among FSWs in sub-Saharan Africa. FSWs (n = 348) attending the Korogocho clinic in Nairobi, Kenya participated from August 2009 to March 2011. HPV DNA was detected using the SPF10-LiPA25 PCR assay. Baseline prevalence of HPV infection and cervical dysplasia were calculated, stratified by HIV-serostatus. Incidence rate (IR) of infection was calculated as number of new infections from baseline over person-months among 160 HPV-negative participants with complete 12-month follow-up. Baseline HPV prevalence was 23.6% for any HPV and 20.4% for high-risk HPV (hrHPV) types. Most prevalent types were HPV52 (10.1%), HPV35 (2.3%), and HPV51 (2.3%). A quarter (24%) of participants were HIV-positive. HPV prevalence was higher in HIV-positive (32.1%) than HIV-negative (20.8%) participants. hrHPV prevalence was higher in HIV-positive (27.4%) than HIV-negative (18.2%) women. During follow-up, HPV IR was 31.4 (95% CI: 23.8–41.5) for any HPV and 24.2 (95% CI: 17.9–32.8) for hrHPV types. HPV52 had the highest IR (6.0; 95% CI: 6.5–10.3). Overall HPV and hrHPV prevalence were lower than expected, but both prevalence and incidence were higher in HIV-positive than in HIV-negative women.
