Abstract Tesim:: Relatively little is known about how commitment manifests in couples’ everyday lives in a way that orients couples toward their future together. Building on the Investment Model of Commitment, we propose that, in everyday life, individuals with high levels of commitment are more likely to (a) assure their partners about the future of the relationship and (b) behaviorally invest in that future by spending more time in the partner’s presence. Results from a sample of individuals (N = 100) suggest that relationship commitment is associated with greater time investment in the relationship (i.e., time spent with the partner in daily life); results from a second sample of couple members (N = 269) replicate this effect and suggest that relationship commitment is associated with the use of daily assurances, which mediate the relationship between commitment and time spent co-present with the partner.

Abstract Tesim:: Dominance and prestige reflect different forms of power and can affect relationship outcomes. Whereas dominance is a conflict-oriented way of dealing with people and grabbing status, prestige is derived from respect and esteem that is granted by others due to superior skills and knowledge. In this research, we tested three partially competing perspectives on potential associations of dominance and prestige with relationship quality (RQ). Sociobiological perspectives suggest that both concepts ensure the provision of valuable resources, and thus predict that both dominance and prestige would be positively linked to RQ. From a sociocultural perspective, men are expected to be dominant and prestigious whereas being dominant (and perhaps being prestigious) would violate the feminine gender-role stereotype; thus, both variables should relate positively to RQ only for men. From a personality perspective, dominance is characterized by undesirable attributes, whereas prestige is characterized by more desirable attributes; as such, only prestige should be positively related to RQ. We conducted four studies with individuals in romantic relationships in three different countries (Germany, Israel, and the US; Ntotal = 2,010 participants). The participants completed measures of dominance-prestige (as general attributes, as relationship-specific attributes, or in comparison with their partner) and measures of RQ. Although not entirely consistent across studies, our results were most consistent with the personality-based perspective. Prestige seems to benefit a relationship for both actors and partners whereas dominance is detrimental to relationship happiness. Yet, as some results showed that the negative link between dominance and RQ was stronger for women than for men, and the positive link between prestige and RQ was weaker for women than for men; thus some weak support for the sociocultural perspective was also found.

Abstract Tesim:: Summary The field of social work has seen increased recognition across many sectors in recent years. Evolving racial, political, and medical paradigms and the COVID-19 pandemic have shown the importance of a person-centered approach to health and mental health. One such area of practice is neurology, specifically work with people with Parkinson's disease (PD), which is the second most common neurodegenerative disease, currently impacting more than 10 million people worldwide. While social workers have long played key roles in multidisciplinary teams in PD care settings, research and literature offering assessment and evidence in this area remain limited. This research study used a mixed-methods approach with additional in-depth qualitative interviews conducted with 11 social workers. This article presents findings from the 11 interviewees who are presently working with people with PD using thematic analysis. Findings Themes emerging were an in-depth detailed role of the social worker, the importance of strong relationships working with people with PD and their families, multidisciplinary teams, and the community. Institutional challenges were highlighted concerning supporting social work positions and encouraging social work intervention from the beginning of diagnosis to the end stages of the disease. COVID-19 proved very challenging for social workers, multidisciplinary teams, and families, and yet positive practices were identified as well. Applications The impact of this study reinforces the essential role specialized PD social workers play in multidisciplinary PD teams and recognition needed to increase their role through early intervention reflected in increasing social work positions in neurology.

Abstract Tesim:: Purpose We sought to describe the prevalence of food insecurity and its relationship with mental health, health care access, and use among lesbian, gay, and bisexual (LGB) adults in the U.S. Design and Setting We analyzed data from the National Health Interview Survey (NHIS), a cross-sectional study of noninstitutionalized adults from all 50 states and the District of Columbia. Sample The study sample was restricted to LGB adults ≥18 years (N = 1178) from the 2021 NHIS survey. Measures Food security was assessed using the 10-item U.S Adult Food Security Survey Module. Study outcomes were mental health (depression, anxiety, life satisfaction, and serious psychological distress), health care utilization, and medication adherence. Analysis Descriptive statistics and linear and generalized linear regressions. Results The study sample consisted of 69% White, 14% Hispanic/Latinx, 9% Black, and 8% people of other races. Approximately half (53%) identified as bisexual and 47% identified as gay or lesbian. Eleven percent were food insecure. Sexual orientation, income-to-poverty ratio, and health insurance were significant correlates of food insecurity. In multivariable analyses, food insecurity was significantly associated with mental illness (including depression, anxiety, and serious psychological distress), limited health care access and use (including inability to pay medical bills, delay in getting medical and mental health care, and going without needed medical and mental health care), and medication nonadherence (including skipping medication, taking less medication, delay filling prescription, and going without needed prescription). Conclusion Food insecurity is a constant predictor of adverse mental health and low medical and mental health care use rates among LGB adults in the United States. Achieving food security in LGB people requires improving their financial and nonfinancial resources to obtain food.

Abstract Tesim:: Purpose Evidence suggests differential impacts of community development, including gentrification and displacement. Public health practitioners and advocates are key stakeholders involved in the community development process related to active living, yet little is known about their perceptions of its impacts. We explored the perspectives of relevant leaders of public health departments and key community and advocacy organizations on community development, gentrification, and displacement. Approach Purposive key informant interviews. Setting CDC State Physical Activity and Nutrition (SPAN) funding recipients. Participants CDC SPAN recipient leadership (n = 10 of 16) and advocacy organizations they partnered with (n = 7 of 16). Method Interviews were recorded, transcribed, coded, and thematically analyzed with direct quotes representing key themes. Results Both groups felt community development held important benefits, specifically by creating healthy living opportunities, but also potentially leading to the displacement of long-time residents. Practitioners reported the benefits were for all community members, whereas advocates noted the benefits were seen in those with privilege, and the consequences were disproportionately seen in disadvantaged communities. Both mentioned the importance and difficulty of getting diverse representation for community engagement. Conclusions Learning how key stakeholders perceive and navigate the community development process can help inform recommendations for better equity in active living community improvements. More work is needed to further elucidate best practices for health and social equity in the community development process.

Abstract Tesim:: Background Around half the US population uses dietary supplements (DS), and concomitant use with medications is common. Many DS include bioactive substances that can interact with medications; therefore, accurate tracking is critical for patient safety. Unfortunately, documentation of patients’ DS use is often missing or incomplete in the electronic medical record (EMR), leaving patients susceptible to potential adverse events. Novel approaches to assist healthcare professionals (HCPs) in capturing patients’ DS use are needed. Objective To assess HCPs’ perspectives on challenges and facilitators of DS documentation in the EMR and their opinions on a proposed mHealth application (app) to aid in DS capture. Methods HCPs, recruited from professional networks, largely in North Carolina, using purposive sampling, took part in semi-structured interviews. We inquired about HCPs’ experiences with DS documentation in the EMR and their opinions about our proposed mHealth app. Interviews were recorded, transcribed, and coded. Thematic analysis included deductive codes based on the interview guide, and inductive codes that emerged during transcript review. Results HCPs (N = 30) included 60% females, mean age 46 ± 10; 70% White. Pharmacists (20%), nurses (17%), and physicians (17%) were the most represented professions. Years in practice ranged from 3–35 years. Most HCPs were concerned about DS safety and potential supplement-drug interactions, and cited several barriers to accurate EMR DS documentation including time constraints, database inconsistencies, and poor patient-HCP communication about DS. HCPs’ views on our proposed mHealth app were generally positive. They expressed that our proposed mHealth app could streamline documentation processes and enhance patient-provider communication. HCPs expressed desire for a high-quality mHealth app that includes access to evidence-based DS information, integrates with the EMR, and does not increase time burdens. Conclusion HCPs believe documentation of patients’ DS use is important but not accurately captured in the EMR. Support was expressed for our proposed barcode-scanning DS mHealth app.

Abstract Tesim:: The lockdowns imposed upon cities, regions, and countries as a response to the COVID-19 pandemic are extraordinary state-sanctioned spatial interventions, both in terms of scale and scope. However, rather than a time-delimited event nor an exceptional circumstance of a temporary crisis, the pandemic lockdown was entangled with long-standing and on-going intimate and embodied histories of political violence, upheaval, militarization, displacement and dispossession. Be it as a result of war, terrorism, natural disaster, or pandemic – lockdown is more than an intervention in physical space and infrastructure alone. It is also an intervention that mobilizes, and often relies on, the sphere of the intimate along different and often unequal geographies of vulnerability. In this Theme Issue, we build on feminist geopolitics and feminist political geography to examine the intimacies of lockdown, seen through the experiences of refugees, migrants, low-income residents, as well as within the contexts of war and terrorism. Here, the politics of embodiment, domesticity and affectivity is central for understanding how lockdowns actively shape and are shaped by intimate geographies, thus advancing the theorization of the lockdown more broadly. The contributions to this Theme Issue gather around the following questions: how does the spatial politics of lockdown mobilize the sphere of the intimate? More broadly, how does the intimate help forge possibilities and places of counter-narratives of solidarity, shared vulnerabilities and care in contrast to renewed militarization, rising authoritarianism, violence, and the expanding spatialities of confinement in everyday life?

Abstract Tesim:: ObjectiveTo analyze the risk and benefit of bowel preparations in elective colo-rectal surgery.BackgroundMechanical bowel preparations (MBPs) have been popularized in colo-rectal surgery since studies in the 1970s, but recent data has called their use into question and examined complication rates between patients with and without bowel preparations.MethodsA retrospective case-review was performed consisting of 1237 elective colo-rectal surgeries performed by two surgeons between 2008 and 2021. Patients received either a MBP, a mechanical bowel preparation with oral antibiotics (OAMBP), oral antibiotics alone (OA), or no bowel preparation; some patients across all categories received an enema.ResultsBowel preparations combined (MBP and OAMBP) totaled 436 patients and showed no statistically significant difference (P > .05) in primary outcomes of wound infection and anastomotic leak when compared to the 636 patients without a bowel preparation and 165 patients with OA. The analysis controlled for comorbidities and presence of enema. Of secondary outcomes, urinary tract infections (UTIs) were significantly more common in patients who received a bowel preparation (P = .047). All other outcomes showed no significant difference between groups, including complications on day of surgery; complications, readmission with and without surgery, and ileus formation within 30 days of surgery; sepsis; pneumonia; and length of stay (LOS). The presence of enemas did not have a statistically significant effect on outcomes.ConclusionsThis study’s data does not support the routine use of MBPs in elective colo-rectal surgery and draws into further question whether MBPs should remain standard of care.

Abstract Tesim:: I began this author’s response sitting on a plush red and pink Chinese blanket; the familiar Himalaya was in view across the Indus River, in Ladakh which is now a Union Territory, not part of Jammu and Kashmir State (J&K) as it had been when I wrote the book. Intimate Geopolitics is the story of the embodied life of territory in this mountainous former kingdom on India’s contested northern borders with Pakistan and China. The book traces how the history of colonization and its post-independence reverberations set the stage for geopolitical life – for decisions about love, friendship, children, and intimate life to become a terrain on which geopolitical meaning is made.

Abstract Tesim:: Objectives To identify differences in practice patterns and outcomes related to the induction of general anesthesia for patients with pulmonary hypertension (PH) performed by anesthesiologists who have completed a cardiothoracic fellowship (CTA group) vs those who have not (non-CTA group). Design Retrospective study with propensity score matching. Setting Operating room. Participants All adult patients with PH undergoing general anesthesia requiring intubation at a single academic center over 5 years. Interventions Patient baseline characteristics, peri-induction management variables, post-induction mean arterial pressure (MAP), and other outcomes were compared between CTA and non-CTA groups. Methods and main results: Following propensity scoring matching, 402 patients were included in the final model, 100 in the CTA group and 302 in the non-CTA group. Also following matching, only cases of mild to moderate PH without right ventricular dysfunction remained in the analysis. Matched groups were overall statistically similar with respect to baseline characteristics; however, there was a greater incidence of higher ASA class (P = .025) and cardiology and thoracic procedures (P < .001) being managed by the CTA group. No statistical differences were identified in practice patterns or outcomes related to the induction of anesthesia between groups, except for longer hospital length of stay in the CTA group (P = .008). Conclusions These results provide early evidence to suggest the induction of general anesthesia of patients with non-severe PH disease can be comparably managed by either anesthesiologists with or without a cardiothoracic fellowship. However, these findings should be confirmed in a prospective study.
