Quality Improvement in Nursing Homes: Evaluating the Quality of a Nurse’s Patient Teaching Near the Day of Discharge to Home

Abstract
  • Transitions in health care, such as discharge from skilled nursing facilities (SNFs) to home can be dangerous by placing older adult patients at risk for avoidable harms. However, SNFs do not commonly standardize the process of preparing patients for transitions in care. Teaching patients and caregivers a complete, written discharge plan on the day of discharge has been shown to improve the patients’ safety during this transition. In this Senior Honor Thesis, a nurse’s re-enactment of the Connect-Home’s day of discharge education was filmed. Using the video and a checklist that was adapted from the Connect-Home intervention study, the components of the discharge teaching were analyzed to determine the quality of the discharge teaching. It was found that the nurse’s teaching was concordant with all components of the Connect-Home teaching protocol. The implication of this finding is that the Connect-Home teaching protocol is feasible and that this nurse’s re-enactment of her use of the protocol can be used to educate other nurses to effectively teach patients at discharge. This is significant because new tools and standardized methods to train nurses on teaching a written discharge plan are needed. It is essential that nurses are trained to consistently use high quality teaching methods, and the protocol demonstrated in this video is an important tool for achieving this goal.
