A Review of the Literature: The Role of Tranexamic Acid in decreasing Maternal Mortality through its Contribution to the Prevention and Treatment of Postpartum Hemorrhage

Abstract
  • Postpartum hemorrhage is the leading cause of maternal death both worldwide and in the United States. The rate of PPH has risen from 7.2 deaths in 1987 to 22 deaths in 2014 per 100,000 live births. The aim of this paper is to review the current literature on using tranexamic acid as a method of treatment and prevention of PPH, to see if tranexamic acid is associated with decreased maternal mortality, and to make suggestions about changing the current recommendations for treatment and prevention of PPH. Twenty-three articles were chosen to be included in this paper. After reviewing the articles chosen, the evidence suggests that prophylactic use of tranexamic acid for PPH reduces blood loss, the need for transfusion, and the incidence of PPH. As for treatment, the evidence suggests that tranexamic acid is an effective treatment for PPH, especially when used as early as possible after diagnosis. WHO has made no recommendations about using tranexamic prophylactically and only recommends the use of TA after uterotonics fail to treat PPH. Therefore, the evidence suggests that changes to the recommendations, such as using TA prophylactically and giving TA as treatment immediately after a diagnosis, should be considered.
